The Grampians Regional Palliative Care Team works closely with palliative care service providers and health care professionals across the region to assist in the ongoing development and delivery of effective palliative care.
The primary role of the team is clinical consultation, providing assistance with complex symptom management. Other elements of our work include education, collaborative strategic planning, preparation of written materials, research and quality improvement processes.
